Print Blank Baptism & Confirmation Form

How can I print out blank Baptism and Confirmation form?
A member was baptized and their family moved out while I was out of the country. The secretary forgot to file the paperwork while I was gone. Now the father has reached out to our ward to see if we can record the baptism for them. The bishop is fine with signing a new form, but I can't seem to print out a blank one since his record is no longer in our system and there's no obvious option to print an empty one. Is there a way to do this other than printing out an existing one and using white-out to allow me to fill in the new data?

I am suspecting this is for a Primary age child. One option it to pull back just the child's membership into your ward, record the baptism and confirmation and print the certificate which can be mailed to the parents once the bishop signs it.

Then send out the child's membership to the new ward, instead of using an address, group with the head of household.

ps If the bishop and the parents did not sign the interview form, you will have to do that part first before anything can be recorded. Once you pull the child's membership back, you can print the form. Mail the form to the parents after the bishop signs it and have them mail it back to you so you can upload it when you record the ordinances.

A blank baptism form can be print at LCR > Membership > Record Ordinance. Select Baptism and Confirmation, then click the Blank Baptism Record Form button/link just to the right of the Print Baptism Record Form button/link. The form can then be printed or the PDF downloaded.
