Melchizedek Priesthood Ordination Record

Is this form available to the the Bishop of the ward for a young man preparing for a mission or does it have to be printed by the Stake Presidency? I am not finding it in LCR. Please help!!!

The reorganization of Leader and Clerk Resources (LCR) last week removed the form from the usual location, or rather how the form is accessed. It can no longer be printed as a completely blank form. The form is found by going to LCR > Reports > Ordinance Record Forms (in the Member Reports section), select Priesthood Ordinance in the "Select an ordinance" drop-down list box, and enter the name of the brother in the "Add or Edit Priesthood Ordination" search box. The system automatically selects the next appropriate Priesthood office. At this point there is a button to print the form with the brother's name and information filled in. Or if the bishop has interviewed the brother and found him worthy to be ordained to the office of elder then enter the interview date and check the box affirming the brother's worthiness. Then the form can be electronically submitted to the stake by clicking the "Submit to Stake" button.

The electronic submission is simple and makes it easy for the bishop to do the electronic submission to the stake without involving the clerk.
