Recording of Restoration of Blessings
After first presidency permission is granted to a stake president to restore a persons priesthood and temple blessings, and that ordinance is complete, how is it reported back to headquarters?

You're not able to find anything poking around in LCR, perhaps poking on options on the member's record?

Since this is just recording of an ordinance, I'm thinking it's more of a membership/ward clerk function. All permissions have already been given.
General Handbook 32.14.4 provides some clues.

If the member was endowed, Church headquarters updates the membership record to show the new baptism and confirmation dates. This record also includes the message “Restoration of Blessings Required.”

That is talking about when they are rebaptized. The Restoration of Blessings happens later. @ckmcdonald: If you're not sure about the paperwork trail (it might have been something the person doing the Restoration of Blessings had), you could call GSD to see what the process is and what your part in that process is.

The Stake President should check under "Membership=>Confidential=>Actions in Progress", where he will have links for all current actions in process.
I know that when a membership council is reconvened for a matter requiring First Presidency approval, an Application to the First Presidency is initiated after the membership council is completed. Then the Bishop is then automatically notified to complete and submit his portion online.
So for the OP which is about restoration of blessings, my thought is that the Stake President should have some communication from the Office of the First Presidency (I'd guess a link in LCR under the "Confidential Actions" section), which the SP would click and then edit with the appropriate information.

I am pretty sure that the stake president himself needs to go into LCR to record the ordinance. It is also likely that with the authorization to perform the restoration, the stake president received instructions on how to record it.
