Patriarchal Blessing Template

I'm just wondering if there's an online Patriarchal blessing template that I can use to type the patriarchal blessings. It is faster to do it on the computer rather than manual typewriter.

Sharing our stake's template. It uses MS Office Word template fields for easier editing. Libre Office Writer can be used to open and edit it (needs manual text selection and replacement, though, with Writer).
