Parental Authorization

My wife is the Primary Activity Day leader. She is looking for a general Parental Authorization for Travel/Activity to use for her class. Anyone have a good one?

Keep in mind that as the Scouting program is the activity arm of the priesthood, so the Cub Scouting program is the activity arm of the Primary. I'm not suggesting that the activity is under the Cub program, only that the BSA also has a release form for their activities.
