Where do we get key fobs for building access? Who creates the fobs and can distribute them?

Both questions are dependent on your stake and your FMG manager. There may be a person assigned in your unit to manage the fobs. Your unit clerk should know who that is. If there is not a unit person then your stake facilities representative, usually a member of the high council, will know who has been assigned to manage the fobs. If he doesn't know he can contact the FMG manager to find out.

Has anyone been able to develop a electronic key management database of sorts other than a spreadsheet, ideally i would really like this to be similar to the Certifications dabase, ie a key is associated with an individual and "checked in or out" when a person is called and released

Are you talking about a physical electronic key, which would be managed like most physical keys? Some buildings are getting an app-based system where keys are based on calling and largely self-manage.
