Under Tools, Missionaries we see 6 Sisters listed as serving in our Ward. In fact only 2 are there. I can't find a way to delete the 'old' ones. I also have no idea how these missionary names get added to our unit?
How can I delete the old ones? Who adds the names in the first place.

The answer to both questions is the mission.
However, you might want to do a data refresh in the app just to make sure the problem isn't with the sync.

Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.

To my knowledge, the mission office system (iMOS) doesn't have a separate page for the office staff to manage the missionary ward assignments that appear in Member Tools specifically. The issue of 'old' missionaries appearing in Tools is quite common, and I think it's a device-side error so doing a data refresh or un/reinstalling Member Tools as mentioned might help. Let the developers know about this bug in the Member Tools app by going to Help > Send Feedback if it persists

Something is going on outside of the local ward to add "Assigned" missionaries to the Missionary section in Member Tools as well as Ward Directory and Map. I'd assume that's something set by the local mission office since the assignments are ward specific. Where it's set, I have no idea. I wouldn't be at all surprised if someone creative soul thought it was a good idea to assign other missionaries to a ward to allow them to see the ward's contact information - not realizing that it messed up what the members saw in Tools and Directory.
Then again, someone might just be falling behind the changes.

I have been in contact with the Global Services Department in the past about how the missionary assignments are made so they appear in Member Tools. The feedback I got was the mission office had the only access to edit the names assigned to each unit. We were to contact the mission office for any discrepancies we encountered. They did not identify the program/app/system used. But any issues we had were rectified by the mission office after we called them.

As a full time missionary serving in the office, I was just looking today and everything is normal on the mission office side but it shows many missionaries from the last transfer also in the areas, including those who returned home.
When I went to missionaries serving from my home stake it displayed some of my friends who I know for a fact have returned home months ago still on the list.
I don't think much is going wrong in mission office as each area in my mission has the correct missionaries assigned to it and yet the information in Tools is still incorrect.

Member Tools most likely is not updating the data correctly. The Member Tools sync sometimes does not function properly. To get the data straightened out, go to the Settings menu and tap Refresh Data. This will download a totally new data set to replace the old data set.
