Recommend to perform an Ordinance

Recommend to perform an Ordinance
The link to get to the recommend to perform ordinances seems to be broken. When I click on "recommend to perform an ordinance" in the General Handbook (18.7.3) it takes me to a page that says "File Not Found." I know that there have been a bunch of changes to Leader and Clerk Resources; is there another way to find this form?

Who enters into LCR the info for a Recommend for Living Ordinances?

I think it isn't in the computer system until someone at the temple puts it in when they go and tear off the living ordinance part. Correct me if I'm wrong.

We are talking two different things in this thread. The recommend to perform an ordinance has nothing to do with temple work. It is for performing a living baptism, confirmation or priesthood ordination outside of your ward or branch.
A bishop or branch president starts the recommend for living temple ordinances. Currently the recommend for being a living participant in a temple ordinance is a two-part paper recommend. It is not done in LCR, at least for now. Once the stake or district president authorizes the recommend it will appear in the list of current recommends in LCR however.
