Photo Permission
Do I use the Participant Release form for photos taken at Stake Girl's Camp? I've heard lots of differing views on what to use and where photos can be posted.

Follow Handbook 2: 21.1.22 Internet, along with the pointer toward internet.lds.org, and you'll have the authoritative instruction.

This resource Use of Online Resources in Church Callings is a good reference doc, and it points to the Participant Release (Release to Use Name, Image, Voice, Likeness, and Performance) form. I think (?) that's the correct form you're looking for, perhaps there's a more "one-time use" version than what this form seems to be.

What is the purpose the form is needed to fulfill? If it is for the unit history the resources for that are found at Unit Histories and Annual Histories. On that page click the Related Forms section to expand the links where there is a link to the Permission to Use Photographs form. If the purpose is for consent from individuals depicted in photos on limited access Church and unit websites then see Photo Upload Guidelines . For this purpose granting consent can be verbal. Public facing website use of photos would most likely require documented consent by signature such as identified in BrianEdwards' post.
